Newport Dental ConfiDent® Plan 2012 for California

The ConfiDent® Plan 2012 offered by Newport Dental is a prepaid plan that provides patients in California with significant savings on dental care. For a $40 annual membership fee, the ConfiDent 2012 Plan offers you reduced fees on many services. If you already have dental insurance, you can use the Confident 2012 Plan for services your insurance does not cover.

Plan Members can:

• Obtain a Referral to a Specialist: Call 1-800-49SMILE.
• Contact Your Dentist After Hours: For emergencies, call your local office.

• Submit an on-line Grievance Form: Sent though our secure server directly to Newport Dental Plan.
  Or, print a PDF-format form.

The Department of Managed Health Care is responsible for regulating health care service plans. If you need help with a grievance involving an emergency, a grievance that has not been satisfactorily resolved by your plan, or a grievance that has remained unresolved for more than 30 days, or if you would like more information on the Department of Managed Health Care, click on http://www.hmohelp.ca.gov.
