Very unhappy with the situation happened with insurance claim that was submitted to my insurance company. Instead of billing my insurance for regular cleaning office submitted claim for dental deep cleaning that was denied by Cigna and I have now outstanding balance of $90 that I have to pay for out of my pocket.

Orthodontics may be provided by general dentists.
