Patient Information: Just a few simple forms detailing relevant medical history, any medications you might be taking, your dental history and any dental insurance coverage you may have. Your general health and well-being is important to your dental health. Some medications can affect the teeth and gums, letting our dentists know about them will help us better understand your needs. Want to get a head start? Download the new patient forms below.
X-rays: If you've had dental x-rays in the last six months, please bring them to your first appointment. Bitewings are an essential tool in helping dentists assess the health and condition of your teeth. If you haven't had x-rays recently, plan on biting down on that little cardboard thingy and having new ones taken.
Financial and Insurance Coordination: Our patient care coordinators will work with you to make the treatment you need affordable, whether or not you have dental insurance. You may be able to complete all of your treatment on the same day, or additional appointments might be necessary. Rest assured that we will do everything in our power to get you the quality care that you need at a price you can afford.
Oral Examination: The dentist will perform a complete examination of your mouth, and will discuss any treatment you may need. Your teeth cleaning may be performed at your first visit, or might be scheduled for a follow-up visit depending on the condition of your teeth and gums. The oral examination can identify conditions such as tooth decay, periodontal disease, and even oral cancer.
Your Bright Now! Dental team is your partner in maintaining good oral health, so you are encouraged to ask your dentist or patient care coordinator if you have any questions. Your first appointment will take between an hour and an hour-and-a-half. Patients under the age of 18 must be accompanied by a parent.
